Medication Permission Form This form is required to be on file with Administration to allow distribuion of medication to your child at school.

General Heath Appraisal Form To be filled out by child's parent and physician regarding the current health of the student. This form is required to be on file with Administration.

Immunization Instructions Immunization requirements for Foothills Academy
Classroom Supply List List of classroom supplies needed by students for the 2010-2011 school year.
Acceptable Use Policy The guidelines for student computer and internet use grades 3-12. AUP must be read and signed by student and parent before a student can use Foothills Academy computers.
Laptop Acceptable Use Policy The guidelines for student personal laptop use during school hours or on FA campus. Laptop AUP must be read and signed by student and parent before a student can use their own laptop at Foothills Academy. Students in grades 9-12 only. Students in 7-8 can request a laptop AUP from their homeroom teacher and are subject to any additional guidelines and expectations set by the middle school. Access to Foothills wireless network will not be provided for students in grades 7-8. Students in 5-6 are prohibited from using personal laptops at Foothills Academy unless approved by school counselor and middle school teachers in advance.
